About the Role
This is an in-person role based on the Sunshine Coast commencing part-time with a clear pathway to full-time as the caseload grows. The position is permanent and offers variety across community home-based and gym-based settings supported by a partner gym facility available for client programs as required.
The caseload will primarily include Support at Home and aged care clients alongside opportunities to work with NDIS participants private clients chronic disease management return to work programs and falls prevention. You will be part of a multidisciplinary team and supported to build referral relationships and develop areas of clinical interest over time.
Key Responsibilities
- Deliver individualised exercise physiology services across community home and gym settings.
- Conduct comprehensive assessments and develop evidence-based exercise programs tailored to client goals medical history and functional needs.
- Provide interventions for chronic disease management mobility and balance falls prevention functional reconditioning and return to work.
- Support aged care and Support at Home clients to improve strength safety independence and participation.
- Deliver services to NDIS and private clients across a range of presentations.
- Maintain accurate clinical documentation in line with professional and legislative requirements.
- Work collaboratively with Physiotherapists Occupational Therapists Dietitians and other team members to support holistic service delivery.
- Build and manage your own caseload demonstrating strong organisation time management and clinical reasoning.
- Contribute to service development community engagement and continuous improvement initiatives.
About You
- Accredited Exercise Physiologist with ESSA (AEP) registration.
- Bachelors degree in Exercise Physiology or equivalent.
- Previous clinical experience required this role is not suited to new graduates.
- Ability to work independently manage a varied caseload and build professional relationships.
- Experience or strong interest in aged care Support at Home NDIS chronic disease and community-based practice.
- Confidence delivering in-person services across home community and gym environments.
- Current drivers licence.
- NDIS Worker Screening Check & WWC or willingness to obtain.
- COVID and influenza vaccination evidence.
- Full working rights in Australia.
- Willingness to travel primarily across the Sunshine Coast with occasional visits to the wider region including North Brisbane and hinterland by mutual agreement.
