Job Summary

Job Title: LTTS Service Care Coordinator

Location: Harlingen La Feria Mercedes Weslaco Donna

Duration: 6 Months (Possible extension)

Shift: 8 AM to 5 PM Monday Friday

PR: $24-26/Hr.

Responsibilities:

Assist in telehealth biannual assessments in person as needed if in the area.
Complete assigned tasks and document findings.
Coordination with multi-disciplinary teams.
Follow up calls to members.
Any other projects that may arise.
Must be able to meet strict turn-around times with documentation for assessments assigned.

Required: Licensed Vocational Nurse (LVN)

Years of experience required: at least 1-2 years case management or home health exp

Microsoft Word Excel experience Computer Literacy

Disqualifiers: No vehicle No internet Performance indicators: Best VS. average: Strong Time management skills and very well organized.

Additional qualities to look for: At least 2 years experience at LVN Home Health experience Case management experience speaks and understands Spanish
