Position Summary
The Adult Behavioral Health Home Coordinator / Community Integration Specialist supports adults in building the skills and accessing the resources needed for successful independent living. This role emphasizes holistic person-centered care guided by SAMHSAs 8 Dimensions of Wellnessemotional social physical financial and more.
Youll work as part of a multi-disciplinary team using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.
What Youll Do
- Build strong therapeutic relationships with clientsincluding those who may be harder to engage
- Help clients set and achieve personalized wellness goals
- Provide skill-building and psychoeducational support (individual & group settings)
- Coordinate care plans and connect clients to essential community resources
- Support clients and families in making informed decisions
- Assist with access to medical behavioral health and social services
- Provide crisis intervention when needed
- Maintain accurate documentation and meet service delivery standards

Work Environment
This is a dynamic community-based role requiring travel to client homes service sites and community locations. Work may be conducted in-person or via telehealth depending on client needs.
Schedule
- Full-time MondayFriday (8:00 AM 5:00 PM)
- Occasional evenings or extended hours as needed
Physical Requirements
- Ability to sit stand walk and use standard office equipment
- Occasionally lift up to 25 lbs
- Travel within the service area required

Requirements
- The Adult Behavioral Health Home Coordinator/Community Integration Counselor requires an associate degree or equivalent credits in a Human Services field.
- Bachelors degree preferred. Must be certified at the full MHRT-C or provisional level. Use of personal vehicle and active drivers license is required.
