Compensation:$28 to $33 with Baylor 8-hour bonus
Shift:630AM to 11PM
Position Summary:
The Licensed Practical Nurse (LPN) in the Health Care Center plays a vital role in providing skilled nursing care and support to residents requiring short-term rehabilitation long-term care and ongoing clinical oversight. This position works closely with the interdisciplinary care team to ensure the health safety comfort and well-being of residents while promoting the highest possible quality of care and quality of life.
Essential Duties and Responsibilities:
- Administer medications and treatments as prescribed by physicians ensuring accurate documentation and adherence to medication administration protocols.
- Perform routine nursing assessments including vital signs physical observations and resident evaluations to monitor health status and identify changes in condition.
- Collaborate with the healthcare team to assist in the implementation and updating of care plans based on each residents medical needs and preferences.
- Provide direct nursing care including wound care catheter care restorative support and assistance with activities of daily living (ADLs) as needed.
- Communicate effectively with residents families physicians and the interdisciplinary team to support a coordinated approach to resident care.
- Serve as a liaison between residents families and healthcare professionals by relaying important information addressing concerns and providing support.
- Collaborate with physicians therapists social workers and other clinical staff to coordinate and integrate care services.
- Document and maintain accurate timely and up-to-date medical records including assessments care plans medication records and progress notes.
- Implement and enforce infection control safety and clinical care protocols to maintain a safe and supportive environment for residents.
- Monitor residents for signs of decline distress or medical emergencies and take appropriate nursing action in accordance with established procedures.
- Respond promptly and effectively to resident needs changes in condition and emergencies.
- Support admissions discharges transfers and related nursing documentation as needed.
- Other duties as required.
Knowledge Skills and Abilities
- High School Diploma or GED.
- Active Licensed Practical Nurse license within the state of Kentucky.
- This can include attending workshops seminars and pursuing additional certifications or specialized training in Health Care Center.
- Ability to provide compassionate and personalized care to residents.
- Effective and clear communication
- Knowledge of medication management including proper dosage administration routes and potential side effects.
- Attention to detail and adherence to legal and ethical standards.
- Willingness to collaborate and work as part of a multidisciplinary team.
