About the Role
We are seeking an experienced Support At Home (SAH) Coordinator with the enthusiasm care and compassion to support people living with dementia based out ofSubiaco Head Office.
This position coordinates the individualized supports and services for our clients from onboarding through to review to enable the clients needs and goals to be met throughout their dementia journey.
Key Responsibilities
- Champion person-centred care particularly for clients living with dementia
- Undertake comprehensive client assessments and develop individualised care plans
- Support client onboarding ensuring strong financial stewardship and program accountability in line with SAH guidelines
- Coordinate services and oversee delivery to ensure continuity of care working closely with scheduling clinical and wider teams
- Monitor client wellbeing through regular check-ins and contribute to health safety and quality improvement activities
- Ensure services are delivered in line with the Aged Care Quality Standards maintaining compliance and best practice
- Provide clear information to clients regarding service options costs budgets rights and responsibilities
- Build and maintain relationships with local referrers including liaison with My Aged Care and assessment teams to grow and manage a client portfolio
About You
- Demonstrated experience in service coordination within aged care disability or community services
- In-depth understanding of Support at Home (SAH) services; experience with CHSP is highly regarded
- Proven ability to assess client needs and develop appropriate care plans
- Strong organisational skills with the ability to plan prioritise and meet deadlines
- Empathy and experience supporting people living with dementia
- Excellent verbal and written communication skills
- Ability to build effective relationships with clients families and internal stakeholders
- Demonstrated commitment to positive client outcomes with integrity and professionalism
- Ability to work both independently and collaboratively within a team
Mandatory Requirements
- Working rights in Australia
- Current National Police Clearance
- Valid WA C Class Drivers Licence and access to a reliable vehicle
Desirable:
- Certificate IV in Aged Care Community Services or a related qualification
