The Service and Team:

The Independence and Wellbeing Service part of Adult Social Care operations is looking to recruit an experienced Service Manager to lead our Acute Hospital Discharge and Reablement Teams.

We would welcome applications from those with significant Adult Social Care experience and either a professionally registered Social Worker or Occupational Therapist.

The Role:

Service Manager - Acute Hospital Discharge and Reablement Service.

This is an excellent opportunity to lead service development and build excellent working relationships with our NHS colleagues. We are looking for someone with excellent knowledge of the Care Act 2014 and its application within the acute hospital (including safeguarding processes) alongside the importance of a reablement first and strengths based approach to practice.

This is a public/customer-facing role where the statutory English language requirement for public sector workers applies.

This position will be subject to anenhancedcriminal record disclosure check.

Working Pattern:

Full time - 37 hours per week flexible working requests considered (e.g. condensed hours) and hybrid working arrangements.

What youll need to succeed:

  • A passion for promoting and supporting the role of social care in the acute hospital sector.

  • Experience of a reablement first approach and ensuring strengths based practice.

  • Ability to work with a variety of stakeholders across health and social care including commissioning colleagues.

  • Significant experience within Adult Social Care and achieving the best outcomes for people who need social care support.
