Job Summary
The Home Infusion Clinical Services Liaison RN is responsible for:
Positive and effective representation of the Mount Sinai Home Infusion Program and introducing the full scope of services offered to hospital case managers physicians provider staff nursing agencies other referral sources and patients and their caregivers.
Providing education about the program and services to referral sources and patients.
Assisting with clinical coordination of care between referral sources the pharmacy nursing agency and patient / patient caregivers on behalf of Mount Sinai Home Infusion.
Providing and utilizing clinical knowledge and skills to assist referral sources &/or patients with:
o The identification of appropriate candidates for home infusion
o Confirming the patients and/or caregivers ability to receive care in the home
o Patient education and training regarding therapy and supplies as well as clarifying expectations of care in the home.
o Medication connection at the hospital (i.e. inotropes) / facility in order to expedite the discharge into the home setting as appropriate
Responsibilities
Develops and maintains core therapy and service line admissions to home infusion through positive referral source relationships and responsive caring customer service.
Identifies and communicates opportunities for expanding the home infusion business among current referral sources and new opportunities.
Demonstrates thorough knowledge of core therapy referrals and qualifying criteria by payer source such as: TPN and Inotropic Agents for Medicare.
Evaluates current and future clinical and service needs for existing and new service areas.
As appropriate &/or required coordinates &/or conducts in-services and continuing education programs for referral sources and their support staff.
Collaborates with referral sources (i.e. hospital case managers home health agencies physician offices)and Mount Sinai Home Infusion staff to facilitate the coordination of care for new patient referrals to Mount Sinai Home Infusion.
Coordinates and communicates all critical workflow activities for new patient referrals to Mount Sinai Home infusion personnel and referral sources including but not limited to:
Intake
Clinical Operations: Pharmacy &/or Nursing
Warehouse Operations if required
Reimbursement if required
Appropriately documents the coordination of care activity including but not limited to:
Home infusion &/or nursing services utilized
All communication with patients referral sources physicians and Mount Sinai Home Infusion personnel
All patient &/or caregiver education & training.
Ensures all documentation is legible accurate and complete and complies with company policies & procedures as well as regulatory and accreditation standards.
Maintains up-to-date referral source records as well as any other requested records in accordance with Mount Sinai Home Infusions policies procedures &/or business goals.
Assist with activities to achieve favorable net revenue and gross profit goals for Mount Sinai Home Infusion.
Represents Mount Sinai Home Infusion pharmacy in a professional and positive manner.
Attend and participate in appropriate office meetings outreach meetings and training programs
Coordination and cross-training of departmental work duties to achieve maximum productivity and efficiency.
Remains current with developments in field of infusion therapy.
Comply with all company policies and directives for the fulfillment of the company objectives
Patient Care
- Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
- Integrates relevant assessment and intervention skills in the delivery of nursing care.
- Collaborates with the interdisciplinary healthcare team in the development implementation and evaluation of the plan of care.
- Prioritizes all aspects of patient care including teaching rounding coaching and planning after hospital care and delegating to others as appropriate.
- Educates patients and caregivers while anticipating needs and readiness to learn about their plan of care transition of care promotion of health and prevention of disease.
- Communicates effectively and professionally with patients family and all members of the Interdisciplinary Patient Care Team.

Qualifications
Education Requirements
Bachelors degree in nursing from an accredited college or school of nursing.
Experience Requirements
Relevant clinical competence in area of nursing practice assigned
Strongly Preferred:
3 years of IV-related experience
2 years of home health care experience
1 year of Case Management experience
Licensing and Certification Requirements (if applicable)
- Licensed as a registered nurse with current registration in New York State
- Name: Basic Life Saver (BCLS) Issuing Agency: AHA
- Certified Registered Nurse Infusion (CRNI) preferred
- Evidence of ability to maintain interpersonal relationships communicate appropriately to others and work effectively and efficiently to solve problems
- Demonstrates the ability to work in a team displaying effective communication and critical thinking
- Ability to read write understand and speak English in a clear and concise manner
- Visual physical auditory and cognitive ability to assess patient status
- Physical and mental health necessary to care for acutely ill patients with a broad variety of diagnoses and behaviors on a daily basis
- Ability to walk stand lift stoop push and respond quickly in a variety of situations and for an extended period of time
- Manual dexterity to perform a variety of clinical procedures relevant to practice area (i.e. venipuncture urinary catheterization cardiopulmonary resuscitation)
- Ability to use basic data entry retrieval systems and the electronic medical record documentation system in a proficient manner
Compensation Statement
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $87692.00 - $131538.00 Annually. Actual salaries depend on a variety of factors including experience education and operational need. The salary range or contractual rate listed does not include bonuses/incentive differential pay or other forms of compensation or benefits.
