Job Summary:
This position is responsible for providing diagnostic and therapeutic services patient/family education and appropriate referrals as necessary to patents; maintaining systematic records in accordance with professional and regulatory guidelines; and maintaining quality care for all patients through interaction with hospital personnel in a coordinated comprehensive multidisciplinary approach to enable everyone to reach his or her maximum level of independence. This will be accomplished by performing the following duties in accordance with Community Hospital and the Grand Valley Rehabilitation Services policies and procedures. This position will cover both acute care and outpatient services.

Responsibilities:
Coordinates and directs patient care services to patients of all ages to ensure that patient needs are met. Services include patient evaluation/ assessment establishing and implementing a plan of care assessing effectiveness of and modifying treatment to ensure goal achievement and educating patients/caregivers.
Demonstrates the ability to apply therapeutic principles and practices based on current evidence and knowledge appropriate clinical judgment and patient preference.
Produces quality documentation in a timely manner to support provision of care according to current standards that are based on professional association and regulatory guidelines.
Demonstrates an understanding of treatment costs and financial support as they relate to quality and efficiency including budgeting staffing billing/coding and procedural planning tasks.
Submits appropriate accurate and timely billing and coding information for services rendered.
Educates others (patients caregivers staff students other health care providers business and industry representatives Community Health Partners) using relevant and effective teaching methods.
Requirements:
Bachelor s degree in physical therapy from an accredited program Doctorate s degree preferred.
Current unrestricted license as a Physical Therapist in the State of Colorado (required)
BLS Certification (must be obtained within 90 days of hire)
