Location: Portland OR
Assignment Length: 13 Weeks
Schedule: Day Shift 5x8 Hours
Shift Timing: 8:00 AM 4:30 PM
Weekly Hours: 40 Hours
Position Overview
Navitas Healthcare LLC is seeking an experienced and compassionate Licensed Master Social Worker (LMSW) for a Home Health travel assignment in East Portland and surrounding communities. This role is ideal for a social worker who is passionate about providing patient-centered care in a home health setting and thrives in an independent field-based environment.
The LMSW will work collaboratively with interdisciplinary healthcare teams to support patients and families through psychosocial assessments care coordination counseling and access to community resources.
Key Responsibilities
- Conduct psychosocial assessments and evaluate patient and family needs in the home setting
- Develop and implement individualized care plans in collaboration with healthcare teams
- Provide counseling emotional support and crisis intervention services
- Coordinate community resources and assist with access to social financial and healthcare services
- Educate patients and caregivers regarding support programs and long-term care planning
- Maintain accurate and timely documentation within electronic medical record systems
- Participate in discharge planning and continuity of care coordination
- Travel to patient homes within assigned coverage areas
- Ensure compliance with agency policies state regulations and professional standards
- Manage daily patient visits efficiently while meeting productivity expectations
Qualifications
- Active Oregon LMSW license required
- Washington LMSW license preferred or currently in process
- Minimum 1 year of Skilled Home Health experience required
- Oasis documentation experience required
- Current BCLS certification required
- Experience with EPIC charting systems preferred
- Valid drivers license reliable transportation and current auto insurance required
- Strong communication organizational and time management skills
- Ability to work independently in a field-based environment
- COVID-19 vaccination series plus one booster required
