Job Summary
Title - LTSS Service Care Manager-RN
Duration - 6 months Possible extension & conversion
Shift - Work schedule 8:00 am 5:00 pm Monday-Friday.
Location - San Antonio TX and surrounding areas - Bexar County and counties that directly touch Bear County.
Pay rate: $40-48/hr
Note -
- Training Monday - Friday 8am-5pm CST 6 week course classroom setting CAMERA ON NO TIME OFF DURING TRAINING
- In-home visits up to 3x a week
- Must have reliable transportation
Responsibilities:
- We completed 6 member visits per week with 2 days for documentation.
- Typically have 3 days with 2 visits per day and 2 full days for documentation.
- Member visits completed in members home so travel from employees home office to members home and back is required.
- We do work with the pediatric population so we are scheduled visits at 3:00pm which have a great potential (with drive home) to go past 5:00 pm.
- This is also a documentation heavy position with specific turnaround times for documentation to be submitted.
- Performance metrics are measured by documentation and training compliance as well as set company goals.
Education Required:
- RN ADN BSN or MSN
License Required:
- RN
Years of experience required:
- More than 3 years nursing experience and 2 years case management experience
Additional qualities to look for:
- Home Health experience prefer candidates with pediatric experience
