Service Coordinator

In this role you will ensure our residents are linked to supportive services that they need to continue to live independently and comfortably. You will coordinate and assist with case management services with a focus on wellness and independent living.

Position Type:Part-Time 54 hours per pay period working a varying schedule

Wage Range:$20 - $26 /hour depending on experience

Service Coordinator Responsibilities:

• Refer and link residents to services in the community including transportation legal advocacy nursing services and wellness screenings.
• Assist residents with tasks such as completing forms or contacting resources.
• Maintain positive working relationship with providers who serve the residents.
• Work with resident council as an adviser including attending meetings taking minutes and assisting as needed between meetings.
• Develop and maintain a strength-based solution-focused approach when working with residents coworkers public and other professionals.
• Coordinate educational opportunities for residents.
• Perform additional tasks as needed.

Service Coordinator Qualifications:

• Bachelors degree or appropriate work experience
• Knowledge of and training in elderly and disability services
• Knowledge of referral processes
• Demonstrated working knowledge of local services for the elderly and people with disabilities
• 2-3 years of social service delivery experience. It is preferable for service coordinators to have direct experience working with elderly people or people with disabilities
