Intensive Care Coordinator - Bachelors Level
Exciting opportunity to join our innovative and growing childrens mental health program as a Bachelor Level Intensive Care Coordinator!
We are seeking a compassionate and culturally sensitive professional who is dedicated to making a positive impact on the lives of others and thrives in a diverse and collaborative environment!

- Provide comprehensive care coordination to children and families.
- Perform initial clinical assessments to identify family and youths need and goals.
- Develop implement and monitor individualized care plans based on family needs and goals.
- Facilitate care plan team meetings.
- Act as a liaison with collateral agencies and programs
- Advocate for families and educate them on self-advocacy
- Identify and assist youth and families in accessing necessary services
- Ensure proper documentation and safety planning
- Participate in staff meetings supervision and training
Schedule: Full Time 40 hours per week
Pay: $22.28/hour

Required Skills
- Excellent communication and organizational skills
- Basic computer fluency including Microsoft Office
- Valid drivers license and reliable vehicle
Required Experience
- Bachelors degree in a mental health-related or human services field (such as social work counseling child development psychology human services) from an accredited college or university
- Minimum of one-year experience working with children with serious emotional disturbance
- Community-based experience strongly preferred
