Job Summary
The Care Management Assistant (CMA) is an essential member of the Integrated Care Management team. The CMA works directly with patients by conducting outreach and assessment of patients during transitions in care and closing care gaps related to quality measures. Additionally the CMA will respond to and complete a wide range of clinical and administrative tasks as directed by the care management team. The CMA will work under the guidance of the care management staff and will collaborate with other Frederick Health departments as well as community partners.

Example of Essential Functions:
- Gaps in Care: Supports value-based care contracts by outreaching to attributed patients generating orders/referrals obtaining medical records and scheduling appropriate appointments to close identified quality measure gaps. Provides education on importance of health maintenance and routine testing to patients.
- Ability to access and utilize multiple software systems to identify target patients and complete required documentation.
- Outreach to patients during care transitions to assess understanding of discharge instructions provide education as appropriate ensure follow up appointments are made and identify barriers that may lead to poor health outcomes and/or readmission.
- Collaborates with primary care team other Frederick Health departments and community partners.
- Navigating Social Determinants of Health (SDOH) barriers by assessing and identifying SDOH related problems and providing information and referral to the appropriate community resources while collaborating with care managers.
- Fosters trust with patients to facilitate engagement with care management
- Responds and takes actions on a wide range of clinical and administrative tasks
- Proficient in scheduling appointments in electronic health record
- Appropriately takes calls and responds timely to messages/tasks from patients and care team.
- Participate in all staff meetings/in-services
- Other duties as assigned
Required Knowledge Skills and Abilities:
- Knowledge of medical terminology
- Ability to provide clinical education on the importance of routine health screenings and education during care transitions that is within the scope of the CMA role.
- Current computer skills including Microsoft Office Products (Word Excel Outlook Teams)
- Demonstrate effective interpersonal and communication skills

Minimum Education Training and Experience Required:
- AA degree with at least 3-5 years of clinical work experience in a healthcare setting preferred
- Certified Medical Assistant or Certified Nursing Assistant certification preferred
- Must have current CPR certification
Patient Contact
Must demonstrate and maintain current knowledge and skills in providing appropriate care/contact for patients in the following age groups:
- XNeonate (0 thru 30 days)
- XInfant (31 thru 12 months)
- XChild (13 months thru 12 years)
- XAdult (18 years thru 65 years)
- XGeriatric (66 years)
If applicable must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status.Possess the ability to interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needed as described in the units/departments policies and procedures.
Physical Demands:
Sedentary - Light Work- Lifting up to 15 pounds on an infrequent basis (less than one lift every three minutes).While work is mostly done sitting a certain amount of walking or standing is often necessary.
Ergonomic Risk Factors:
Repetition:Repeating the same motion over and over again places stress on the muscles and tendons.The severity of risk depends on how often the action is repeated the speed of the movement the required force and muscles involved.
Forceful Exertion:The amount of physical effort required to perform a task (such as moving patients) or to maintain control of equipment or tools.The amount of force depends on the type of grip the weight of an object or person body posture the type of activity and the duration of the task.
Awkward Posture:Posture is the position your body is in and its effect on the muscle groups that are involved in the physical activity.Awkward postures include repeated or prolonged reaching twisting bending kneeling squatting working overhead with your hands or arms or holding fixed positions.
Contact Stress:Pressing the body hard on sharp edges can result in placing too much pressure on nerves tendons and blood vessels.For example using the palm of your hand to hit or stick objects can increase risk of musculoskeletal disorders.

Pay is based on experience skills and education. If position is part-time salary will be pro-rated based on scheduled hours. The pay range may also vary within the stated range based on specialty if applicable. Non-Exempt positions may have shift differential and/or Overtime paid if applicable.
Hourly range: $18.34 - $25.60
