Orientation $257.50-262.50 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly $Per hour not OT

Holiday $772.50-787.50 - Must be 1.5x the hourly rate. Per holiday hour no OT

Nightly Call $$1300-$2100 - Per night call to hold the pager to include 2.5 hours gratis

Holiday Nightly Call - $Per night call to hold the pager to include 2.5 hours gratis

On Call (Daily) $Per 24 hours to include 4 hours gratis

24 Hour Holiday Call Rate - $Per 24 hours to include 4 hours gratis

Call Back $$515-525 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Average Callback: Weekday: 7.5 hours; Weekend: 5.9 hours

Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less
