Are you a compassionate and detail-oriented Registered Nurse (RN) looking to make a meaningful impact in home healthcare Excel Home Care Services is currently seeking a dedicated RN to join our team and provide Medication Management to clients in the comfort of their homes.
Position Overview
As a Medication Management RN youll play a vital role in helping patients safely manage their medications. Through in-home visits youll assess needs educate clients and caregivers and collaborate with physicians and pharmacists to support the overall care plan.
Key Responsibilities
- Conduct comprehensive in-home assessments to manage and review medications
- Educate clients and caregivers on proper usage side effects and compliance
- Pre-fill and organize medication planners as needed
- Monitor for adverse reactions and report changes in condition
- Coordinate with the care team to address any medication-related concerns
- Accurately document visits care provided and patient progress
- Perform health assessments including vital signs and ongoing care needs
- Ensure compliance with healthcare standards and agency policies
Qualifications
- Active RN license in the state (Required)
- Experience in home health medication management or geriatric care (Preferred)
- Excellent communication and organizational skills
- Ability to work independently and provide empathetic patient-centered care
- Reliable transportation for home visits
