Personal Support Worker (Part Time Evening Shift)

Yee Hong is one of the foremost providers of culturally appropriate senior care in the country delivering high-quality resident experience and care to Chinese and other Asian seniors since 1994. The organization operates 805 long-term care beds and 308 seniors apartment units with an additional 224 long-term care beds opening in a new home by June 2026. In addition Yee Hong offers four campuses of managed care a hospice residence and a wide range of community-based senior and caregiver support services. Through this comprehensive network the organization serves more than 15000 individuals annually across the Greater Toronto Area.
The Personal Support Worker (PSW) will provide personal care to assist clients in their daily activities and personal hygiene. The PSW will support the clients physical social and emotional needs in a safe manner in accordance with the organizations resident-centred approach complying with relevant government standards residents rights and the organizations mission and philosophy.
Responsibilities:
Assists in the nursing process by observing and reporting changes in the residents physical and emotional condition; provides input into the care plan and implements the guidelines in the plan
Adheres to the legislation standards policies and procedures of the LTC Home Delivers personal care to promote comfort and safety for the resident during activities of daily living in accordance with the standards of resident care set out in the Nursing Manuals
Provides cognitive social emotional support to residents.
Promotes independence of resident in his/her activities of daily living. Observes and respects residents individual rights and choices. Acknowledges the residents need for dignity and Privacy
Assists the resident in the proper use and care of adaptive/prosthetic devices Maintains effective communication and good relationship with residents families and significant others
Participates as a team member in co-operation with other departments to ensure quality care of the resident. Provides input into the plan of care
Documents accurately and legibly on flow sheets and daily records reflecting each residents condition care and specific needs
Assists in the care of residents personal belongings
Complies with risk management and quality improvement programs including infection prevention and control occupational health and safety incident and falls etc. Assists in the care of LTC home equipment by regular cleaning and inspection and prompt reporting of defects.
Provides job shadowing or mentorship to members of the health care team or students as assigned
Practices nursing restorative care to complement and supplement rehabilitation care
Keeps up-to-date with developments and best practices in the field
Other duties as assigned
Qualifications
Graduate of an approved Personal Support Worker course in Ontario or equivalent
Experience in long term care setting an asset
Current CPR Certificate
Registration with Health and Supportive Care Providers Oversight Authority (HSCPOA) preferred.
Resident-centred care focus with strong customer service and interpersonal skills
Demonstrated ability to take initiative problem-solve and time manage
Ability to work independently as well as be an active positive team player
Behavioural management training (Gentle Persuasive Approach (GPA) U-First CPI) an asset
Computer literacy (Point Click Care MS Office). Basic knowledge of Workday an asset.
Ability to speak and converse in Chinese an asset
Additional Information
Working Conditions:
Considerable standing walking lifting and bending
Exposure to all resident care elements through regular resident contact.
Ability to work permanent evening shifts including weekends and short shifts. May be required to work an alternate shift(s) to meet operational needs (e.g. emergency training accommodation etc.) as per scheduling practices
