The Licensed Practical Nurse (LPN) provides direct patient care to residents in a long-term care setting while supporting nursing operations and supervising nursing assistants. This role ensures high-quality patient-centered care in compliance with clinical standards facility policies and regulatory requirements.
Key Responsibilities
Deliver direct nursing care in accordance with care plans and physician orders
Supervise and support daily activities of nursing assistants
Administer medications and treatments safely and accurately
Monitor assess and document resident conditions and responses to care
Transcribe and implement physician orders into medical records and care plans
Communicate resident status updates with physicians and interdisciplinary teams
Perform clinical procedures including catheterization tube feedings suctioning wound care and specimen collection
Record vital signs and maintain accurate clinical documentation
Support resident admissions transfers and discharges with required documentation
Ensure proper inventory and availability of medications supplies and equipment
Provide education and support to residents and family members
Participate in facility surveys inspections and quality improvement initiatives
Maintain confidentiality and uphold resident rights at all times
Collaborate with the healthcare team to ensure coordinated care delivery
Perform additional duties as assigned
Required Qualifications
Graduate of an approved LPN nursing program
Active unrestricted LPN license in Massachusetts
Current BLS / CPR / AED certification
Strong knowledge of long-term care clinical practices and regulations
Ability to supervise staff and make independent clinical decisions
Excellent communication and interpersonal skills
Ability to handle complex and sensitive situations with professionalism
Experience
Prior experience in long-term care or skilled nursing preferred
Previous supervisory experience with nursing assistants strongly preferred
