Job Title: Licensed Practical Nurse (LPN) Long-Term Care (Travel Assignment)
Location: Falmouth MA 02540
Duration: 13 Weeks
Shift: 3:00 PM 11:30 PM
Position Overview
The Licensed Practical Nurse (LPN) provides direct patient care to residents in a long-term care setting while supporting nursing operations and supervising nursing assistants. This role ensures high-quality patient-centered care in compliance with clinical standards facility policies and regulatory requirements.
Key Responsibilities
- Deliver direct nursing care in accordance with care plans and physician orders
- Supervise and support daily activities of nursing assistants
- Administer medications and treatments safely and accurately
- Monitor assess and document resident conditions and responses to care
- Transcribe and implement physician orders into medical records and care plans
- Communicate resident status updates with physicians and interdisciplinary teams
- Perform clinical procedures including catheterization tube feedings suctioning wound care and specimen collection
- Record vital signs and maintain accurate clinical documentation
- Support resident admissions transfers and discharges with required documentation
- Ensure proper inventory and availability of medications supplies and equipment
- Provide education and support to residents and family members
- Participate in facility surveys inspections and quality improvement initiatives
- Maintain confidentiality and uphold resident rights at all times
- Collaborate with the healthcare team to ensure coordinated care delivery
- Perform additional duties as assigned
Required Qualifications
- Graduate of an approved LPN nursing program
- Active unrestricted LPN license in Massachusetts
- Current BLS / CPR / AED certification
- Strong knowledge of long-term care clinical practices and regulations
- Ability to supervise staff and make independent clinical decisions
- Excellent communication and interpersonal skills
- Ability to handle complex and sensitive situations with professionalism
Experience
- Prior experience in long-term care or skilled nursing preferred
- Previous supervisory experience with nursing assistants strongly preferred
