Overview:
The Medical Case Manager (MCM) will provide all aspects of care coordinationâfor the AIDS Institute (AI) Advancing Access Case Management clients: complete initial enrollment (all required AI and program forms/assessments) develop care plans promote compliance with appointments and treatment regimens and encourage self-sufficiency.âThe MCM will assist clients to access services such as primary care substance use treatment mental health counseling transportation intensive care management housing and vocational/educational resources through phone calls to clients and providers and home visits when this role the MCM will serve as a community-based advocate when meeting the client needs. The MCM will also participate in outreach events as needed.
Responsibilities:
- Provide supportive case management to AI clients with a focus on providing access to and support for those who are out of care and/or have an unsuppressed viral load.
- Provide care coordination on an ongoing basis with other service providers and participate in medical case conferences to promote integratedâcare.â
- Serve as the primary point of contact for clients ensuring seamless coordination and communication between healthcare providers social services and community organizations.
- Assist clients in accessing and adhering to medical treatment including antiretroviral therapy (ART) laboratory tests and specialist consultations.
- Follow up with the client and the referral provider to ensure that appointments areâkeptâand that the patient is re-engaged in care and referred toâneeded treatment/prevention.ââ
- Conduct Hep C testing as needed
- Perform various administrative duties including updating the client list and outreach logs; assisting with outreach calls and scheduling intakes and follow-ups.ââ
- Maintain essential data for reporting and program evaluationâin eICare including program enrollment progress notesâcare plansâreferral activity and updating contact information.ââ
- Complete UND enrollments and coordinate referrals for mental health services substance abuse treatment housing assistance legal aid and other support services.
- Provide comprehensive education on HIV/AIDS treatment options prevention strategies and healthy lifestyle choices.
- Promote sexual health and wellness through the use of risk-neutral approaches and evidence-based interventions.
- Build and maintain resource lists for external services and emergency services (i.e. housing domestic violence food pantry)â
- Participate inâscheduled supervisory sessions with the Assistant Program Director and other Departmental Leaders as needed
- Participation in youth community events such asâkikiâballs LGBT events testing events etc.â
- Attend IDT meetings and daily huddles and complete updated info by due date in preparation for meeting.
- Facilitate groups and EBIs and complete documentation within 48 hours.
- Document all client interactions interventions and progress notes in accordance with program guidelines.
Other functions that may be required with this position include:
- Participate inârecurringâinteragency meetings as well as external community meetings to improve integrated care andâmaintain relationships with community partnersââ
- Participate in department activities such as new staff onboarding YPS Pride Committee CQI effortsâetcâ
- Participate in Housing Works advocacy events (minimum 3 in person per year 4 via other mode July-June)â
- Perform other duties as assigned
Requirements:
- Bachelors Degreeârequired plusâ2 years of caseâmanagement experience preferred
