Shift Options: 12.5-hour shifts (4 shifts/week)

• Day Shift: 6:00 AM 6:30 PM
• Night Shift: 6:30 PM 6:00 AM

Job Overview

Navitas Healthcare LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join a long-term care facility in Flagstaff AZ. This role involves delivering high-quality nursing care to residents supporting daily clinical operations and ensuring compliance with all regulatory standards. The ideal candidate will bring strong clinical judgment compassion and experience in a geriatric care setting.

Key Responsibilities

• Administer medications and treatments as prescribed within LPN scope of practice
• Perform clinical procedures such as wound care tracheostomy care catheter care and enteral feeding support
• Monitor and evaluate resident responses to treatments and medications
• Ensure proper medication availability and maintain adequate supplies
• Participate in shift handoffs and communicate critical patient updates
• Collaborate with residents families and interdisciplinary care teams
• Complete initial nursing assessments for new admissions
• Assist with MDS assessments and individualized care planning
• Accurately document all care observations and resident progress
• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
• Attend staff meetings and contribute to care discussions
• Follow infection control safety and regulatory guidelines
• Respond promptly and effectively to medical emergencies

Qualifications

• Active LPN license in Arizona or a compact state
• Prior experience in long-term care or geriatric nursing preferred
• Strong knowledge of medication administration and clinical procedures
• Experience with wound care and basic nursing treatments
• Understanding of geriatric care principles and age-related conditions
• Excellent communication and interpersonal skills
• Ability to assess patient conditions and respond to emergencies
• Strong documentation and organizational skills
• Ability to work independently and as part of a team
• Compliance with all healthcare regulations and facility policies
