Job Overview
Navitas Healthcare LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join a long-term care facility in Flagstaff AZ. This role involves delivering high-quality nursing care to residents supporting daily clinical operations and ensuring compliance with all regulatory standards. The ideal candidate will bring strong clinical judgment compassion and experience in a geriatric care setting.
Key Responsibilities
- Administer medications and treatments as prescribed within LPN scope of practice
- Perform clinical procedures such as wound care tracheostomy care catheter care and enteral feeding support
- Monitor and evaluate resident responses to treatments and medications
- Ensure proper medication availability and maintain adequate supplies
- Participate in shift handoffs and communicate critical patient updates
- Collaborate with residents families and interdisciplinary care teams
- Complete initial nursing assessments for new admissions
- Assist with MDS assessments and individualized care planning
- Accurately document all care observations and resident progress
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
- Attend staff meetings and contribute to care discussions
- Follow infection control safety and regulatory guidelines
- Respond promptly and effectively to medical emergencies
Qualifications
- Active LPN license in Arizona or a compact state
- Prior experience in long-term care or geriatric nursing preferred
- Strong knowledge of medication administration and clinical procedures
- Experience with wound care and basic nursing treatments
- Understanding of geriatric care principles and age-related conditions
- Excellent communication and interpersonal skills
- Ability to assess patient conditions and respond to emergencies
- Strong documentation and organizational skills
- Ability to work independently and as part of a team
- Compliance with all healthcare regulations and facility policies
