Navitas Healthcare LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join a long-term care facility in Flagstaff AZ. This role involves delivering high-quality nursing care to residents supporting daily clinical operations and ensuring compliance with all regulatory standards. The ideal candidate will bring strong clinical judgment compassion and experience in a geriatric care setting.
Key Responsibilities
Administer medications and treatments as prescribed within LPN scope of practice
Perform clinical procedures such as wound care tracheostomy care catheter care and enteral feeding support
Monitor and evaluate resident responses to treatments and medications
Ensure proper medication availability and maintain adequate supplies
Participate in shift handoffs and communicate critical patient updates
Collaborate with residents families and interdisciplinary care teams
Complete initial nursing assessments for new admissions
Assist with MDS assessments and individualized care planning
Accurately document all care observations and resident progress
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Attend staff meetings and contribute to care discussions
Follow infection control safety and regulatory guidelines
Respond promptly and effectively to medical emergencies
Qualifications
Active LPN license in Arizona or a compact state
Prior experience in long-term care or geriatric nursing preferred
Strong knowledge of medication administration and clinical procedures
Experience with wound care and basic nursing treatments
Understanding of geriatric care principles and age-related conditions
Excellent communication and interpersonal skills
Ability to assess patient conditions and respond to emergencies
Strong documentation and organizational skills
Ability to work independently and as part of a team
Compliance with all healthcare regulations and facility policies
