Palliative Care Intake Coordinator
Position Purpose:
The Palliative Intake Coordinator collaborates with hospital discharge planners/social workers liaison personnel physicians billing specialists patients families and others to receive information which facilitates admission of Palliative patients; documents appropriate demographic and clinical information for the processing of incoming referrals; coordinates communication process between referral sources third party payors and professional/clerical staff; and coordinates referral process from initiation to completion including all follow-up necessary to accomplish expected outcome.
Minimum Qualifications:
- Education: High School diploma/GED
- Experience: Minimum of one year recent Hospice experience -- or related setting such as home health hospital ambulatory care center physician office in which assessment and critical decision-making skills are required
- Effective Communication Skills
- Demonstrated ability regarding critical thinking skills telephone and direct assessment of patient responding to emergency referrals establishing priorities re: scheduling admissions of Palliative patients in all settings-home hospital nursing home and assisted living center.
- Effective Interpersonal skills demonstrates tact judgment and diplomatic business-like manner in dealing with physicians other healthcare professionals and coworkers
- Intermediate to advanced Excel skills
