Registered Nurse (RN) Home Health Case Manager

What Youll Do

As a Home Health RN youll be the heart of patient carebringing clinical excellence directly into patients homes.

• Manage a dynamic caseload of patients in your local community

• Perform comprehensive assessments and develop individualized care plans

• Provide hands-on skilled nursing care and patient education

• Monitor patient progress and adjust care plans as needed

• Collaborate with physicians therapists and interdisciplinary teams

• Be a trusted partner for patients and families throughout their care journey

What Youll Need

• Active unrestricted RN license (Connecticut)

• At least 1 year of clinical nursing experience

• Ability to travel locally (youll spend time in the fieldthis is not a desk job)

• Valid drivers license reliable vehicle and current auto insurance

• Ability to safely assist patients (lifting positioning etc.)

• Sit stand bend lift and move intermittently and be able to lift 50100 lbs.

Preferred (but not required):

• Home Health or Hospice experience

• Familiarity with HomeCare HomeBase

• Associates or Bachelors Degree in Nursing
