Schedule: Day Shift 07:30 AM 04:00 PM
Hours: 32 Hours/Week (4 Days/Week)
Pay Rate:$45 - $49/hour (Travel Rate)
Position Overview
Navitas Healthcare LLC is seeking a Licensed Master Social Worker (LMSW) with experience in case management to support patient care in an acute care hospital setting. This role focuses on discharge planning care coordination and providing psychosocial support to patients and their families. The ideal candidate will collaborate with interdisciplinary teams to ensure safe efficient transitions of care while addressing patients social and emotional needs.
Key Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care and discharge plans
- Coordinate patient care across multidisciplinary teams
- Facilitate timely and safe discharges including post-acute care arrangements
- Provide counseling crisis intervention and emotional support to patients and families
- Collaborate with physicians nurses and allied health professionals
- Connect patients with community resources and support services
- Maintain accurate and timely documentation in the electronic medical record system
- Ensure compliance with hospital policies procedures and regulatory standards
- Support care coordination across multiple units as needed
Required Qualifications
- Minimum of 1 year of acute care case management experience
- Strong background in discharge planning and care coordination
- Experience with Med-Surg ED or complex case management workflows preferred
- Ability to thrive in a fast-paced hospital environment
- Excellent interpersonal communication and organizational skills
Education Requirements
- Masters Degree in Social Work (MSW) Required
Certifications & Licensure
- Active Washington State LMSW License (or pending)
- Basic Life Support (BLS) Certification Required
