Position Summary
A well-established not-for-profit community hospital is seeking aFamily Medicine Physician (MD/DO) to join its outpatient primary care practice in Shelton Connecticut. This is a full-time permanent opportunity focused on delivering comprehensive patient-centered care in a collaborative clinical setting.
The ideal candidate will have a strong background in primary care preventive medicine and chronic disease management. Providers will treat a diverse patient population across the lifespan and manage a broad range of conditions including diabetes hypertension asthma and other acute and chronic illnesses.

Key Responsibilities
Provide comprehensive outpatient primary care services including evaluation diagnosis and treatment
Perform routine physical exams preventive screenings and health assessments
Manage acute and chronic conditions such as diabetes hypertension asthma and infections
Order and interpret diagnostic tests including laboratory work and imaging
Prescribe medications and develop individualized treatment plans
Educate patients on disease prevention wellness and lifestyle modifications
Collaborate with specialists and multidisciplinary teams for coordinated care
Maintain accurate and timely documentation within the EMR system
Participate in quality improvement initiatives and patient care programs
Qualifications
MD or DO from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Active Connecticut medical license (or eligibility)
Open to both experienced physicians and recent graduates
Strong clinical judgment communication and patient care skills
