Position Summary
A well-established not-for-profit community hospital is seeking a Family Nurse Practitioner (FNP) to join its outpatient primary care practice in Shelton Connecticut. This is a full-time permanent opportunity focused on delivering high-quality patient-centered care in a collaborative clinical environment.

The ideal candidate will have a strong foundation in primary care preventive medicine and chronic disease management. Providers will care for a diverse patient population across the lifespan and manage a wide range of conditions including diabetes hypertension asthma and other acute and chronic illnesses. Both experienced nurse practitioners and new graduates are encouraged to apply.

Key Responsibilities
Provide comprehensive outpatient primary care services including evaluation diagnosis and treatment
Perform routine physical exams preventive screenings and health assessments
Manage acute and chronic conditions such as diabetes hypertension asthma and infections
Order and interpret diagnostic tests including laboratory work and imaging
Prescribe medications and develop individualized treatment plans
Educate patients on disease prevention wellness and lifestyle modifications
Collaborate with physicians and multidisciplinary teams to ensure coordinated care
Maintain accurate and timely documentation within the EMR system
Participate in quality improvement initiatives and patient care programs

Qualifications
Masters or Doctorate degree in Nursing from an accredited program
Board Certification as a Family Nurse Practitioner
Active Connecticut APRN license (or eligibility)
Open to both experienced providers and new graduates
Strong clinical judgment communication and patient care skills
