Job Summary
Halcyon Home is hiring an In-Office Home Health RN Case Manager to support our growing geriatric population in Austin TX. We offer competitive pay 5 weeks of PTO plus 9 holidays per year and wonderful team support!

- Supervise and support clinical staff including nurses and therapists
- Review and approve patient assessments care plans and documentation
- Ensure compliance with Medicare Medicaid state and accreditation standards
- Coordinate patient care and clinical services with physicians and interdisciplinary teams
- Monitor quality assurance and performance improvement activities
- Assist with staff training scheduling and performance evaluations
- Support survey readiness and audit preparation
- Address clinical concerns and provide problem-solving support to staff

- Graduate of an accredited nursing program (Diploma ADN or BSN)
- Active unencumbered RN license (state or compact/NLC license)
- Minimum 2 years of clinical nursing experience and 1 year of home health experience
- Management experience preferred
- Current drivers license CPR certification and reliable transportation with auto insurance
