Job Title: Licensed Practical Nurse (LPN) Day Shift
Location: Phoenix AZ 85001
Assignment Type: 13-Week Contract (Temp-to-Perm Opportunity)
Schedule: Day Shift 12-Hour Shifts (06:00 AM 06:30 PM)
Weekly Hours: 3640 hours (Alternating: 36 hours one week 40 hours the next)

Position Overview

Navitas Healthcare LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care in a geriatric and long-term care setting. The ideal candidate will support clinical operations administer treatments and collaborate with an interdisciplinary team to ensure optimal patient outcomes and regulatory compliance.

Key Responsibilities

• Administer medications via oral enteral and parenteral routes as prescribed
• Perform clinical procedures such as wound care tracheostomy care and catheter care
• Monitor patient conditions and promptly report changes or responses to treatment
• Maintain accurate and timely clinical documentation
• Assist with admissions assessments MDS processes and care planning
• Ensure proper inventory and availability of medical supplies and medications
• Communicate effectively with patients families and healthcare team members
• Participate in shift reports to ensure continuity of care
• Support Quality Assurance and Performance Improvement (QAPI) initiatives
• Attend staff meetings and contribute to a collaborative team environment

Required Qualifications

• Graduate of an accredited Practical Nursing program
• Active Licensed Practical Nurse (LPN) license in the State of Arizona
• Current Basic Life Support (BLS) certification

Preferred Experience & Skills

• Prior experience in geriatric or long-term care settings
• Strong clinical and patient assessment skills
• Ability to respond effectively in emergency situations
• Excellent communication and interpersonal abilities
• Strong organizational and multitasking skills
• Ability to work independently as well as part of a team
