Position: Intake Case Manager The Cradles Project
Reports to: Program Coordinator
Hours: 37.5 hours per week (occasional evenings and/or weekend hours may be required)
Travel: Office and various community areas (Houston & surrounding areas)
Status: Salary non-exempt
The Council on Recovery is a 501(c)(3) not-for-profit organization whose mission is to keep our community healthy productive and safe by providing services and information to all who may be adversely affected by alcohol drugs and related issues.
POSITION SUMMARY
The Intake Case Manager provides community-based trauma-informed case management services to new and expecting parents impacted by behavioral health challenges including substance use/misuse. This role focuses on conducting screenings intakes assessments and service plans to identify both clinical and non-medical drivers of health such as housing instability food insecurity transportation barriers and trauma. The (position title) works collaboratively with staff and families to develop individualized service and recovery plans connect clients to resources and provide short-term goal-oriented support to promote family stability parental well-being and healthy child development.
EDUCATION AND REQUIREMENTS
Bachelors degree in social work or another related field
LCDC-I; or LCDC (strongly preferred)
Bilingual (strongly preferred)
Minimum of two years in the field of Behavioral Health to include substance use experience
Minimum of two years case management experience
Previous experience working with vulnerable families/teens in the substance use field
Working knowledge of client/community resources and experience in developing relationships
Experience leading groups and teaching to vulnerable client populations
Experience working independently and collaboratively as part of a team
ABLITIES AND SKILLS
- Experience conducting screenings intakes assessments and service plans
- Strong communication and engagement skills including motivational interviewing
- Ability to work effectively with diverse populations using a trauma-informed culturally responsive approach
Proficiency in electronic health records (EHR) or case management systems communication skills both written and verbal
Good decision-making skills
Ability to work as a team member
Able to conduct himself/herself in a professional manner
Needs reliable transportation
Valid and current automobile insurance required for all vehicles driven for work purposes
