The Mount Sinai Hospital is a 1134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical emotional and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907 the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education professional development and extra-curricular opportunities.

Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health Systems implementation of population health initiatives that is programs of care that enhance the patient experience and health status improve the health of patient populations and reduce utilization and healthcare costs. Population health initiatives particularly focus on preventive care. MSHP provides the infrastructure to support population health initiatives and includes services such as care coordination workflow optimization physician engagement and quality Department of Social Work Services (The Mount Sinai Hospital) takes a leadership role in MSHP Care Management which provides in person and telephonic care management services for a large diverse group of individuals who have a variety of insurances that are part of value-based contracts. MSHP Care Management engages in a patient-centered approach which aims through improved care coordination and service integration to control future health care costs and improve health outcomes for this population.

Mount Sinai Health Partners Care Management seeks an Associate Director to have accountability for the quality effectiveness and efficiency of all program initiatives. These initiatives involve the targeted outreach and engagement of high risk members and the provision of psychosocial interventions that increase connectivity to appropriate providers and services thereby reducing the risk of avoidable inpatient and emergency department utilization.Reporting to the Vice-President of Care Management the Associate Director will have three direct reports (supervisors) and will provide the clinical direction for all Social Workers and Care Coordinators (30 FTEs). This individual will also ensure seamless collaboration with their nursing counterpart who is accountable for the work of our Nurse Clinical Coordinators on the Care Management team. The Associate Director will be a critical member of the senior leadership team within MSHP Care Management and will serve as a key point of contact within Mount Sinai Health System and its affiliated practices and payer partners.

This is a hybrid position (remote and onsite work) and a part of the leadership coverage team.



Responsibilities
  • At the direction of the Vice-President of Care Management for Population Health coordinate oversight of clinical and administrative functions of the care management program.
  • Accountable for the performance of three supervisors and their teams of social workers and care coordinators.
  • Helps develop strategy in support of program goals and objectives in alignment with policy and regulatory compliance requirements.
  • Provides decision support by preparing materials progress reports and data analyses for active projects and initiatives under consideration.
  • Assists and advises the VP in implementing operational policies and procedures and ensures operational efficiency is maintained throughout the program.
  • Advises the VP of operational issues related to program activities and recommends strategic solutions as appropriate.
  • Reviews projects in progress and/or at completion to evaluate effectiveness ensure compliance accuracy and timeliness.
  • Participates in internal operational and leadership meetings where future planning data review discussion of issues etc.
  • Contributes to special projects and performs other duties as needed.


Qualifications
  • NYS LCSW.
  • At least 7-10 years of post-MSW experience.
  • Experience supervising social work staff within a health care setting.
  • Demonstrated leadership experience in innovative programs with complex operational and regulatory requirements.
  • Exceptional collaborative organizational and communication skills to meet the challenges of a complicated evolving program.
  • Ability to effectively liaise and work with a variety of internal and external stakeholders in the pursuit of programmatic goals and deliverables.
  • Ability to review and analyze complex data relating to care management activities and initiatives.
  • Proficiency in data analysis/management and related IT systems.
  • Excellent written and verbal communication; able to summarize analyses in a way that simplifies complex ideas and synthesizes information into insights.
  • Minimum of one year in current position.

Compensation Statement

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $95450.00 - $143175.00 Annually. Actual salaries depend on a variety of factors including experience education and operational need. The salary range or contractual rate listed does not include bonuses/incentive differential pay or other forms of compensation or benefits.
