DEPARTMENT SUMMARY:As the nations largest and most experienced provider of LGBT medical and mental health services we help
LGBTQ people lead healthier happier lives by keeping their minds and body well. All services are free or low cost. Were also one of the
few Federally Qualified Health Centers in the nation with providers who specialize in primary care for LGBTQ people and with a research
team working to advance the care and treatment of people in our community.
JOB SUMMARY:The Specialty Referral Coordinator facilitates referrals by collaborating with healthcare providers and the care team.
This position organizes patient information obtains authorizations notifies patients scheduling maintaining documentation retrieving
and indexing consult notes and ensuring timely access to care. Strong communication attention to detail and verifying insurance
coverage are essential for maintaining continuity of care.
ESSENTIAL FUNCTIONS:
Care Coordination
Serve as part of a multidisciplinary care team to provide and facilitate referrals for specialty care and consultants and update the
healthcare team regularly on referral statuses insurance issues and any barriers to patient care.
Process all specialty referrals from primary care providers or other healthcare professionals in a timely manner.
Monitor the status of all specialty referrals ensuring that patients attend appointments and that outcomes are documented in
the patients electronic health record.
Communicate with specialty offices to schedule appointments facilitate required documentation and follow up on any pending
or incomplete referrals.
Verify patient insurance coverage for specialty services obtain prior authorizations and ensure that referrals comply with
insurance requirements.
Collaborate with the eligibility and benefits screening team to trouble shoot eligibility issues and resolve barriers.
Work closely with primary care providers specialty care teams and case managers to facilitate seamless care coordination and
improve patient outcomes.
Maintain accurate and up-to-date records of all referrals in the electronic health record (EHR) system documenting all
communications and actions related to the referral process.
Patient Communication & Advocacy
Contact patients to inform them of referral details schedule appointments with specialists and provide any necessary
instructions or preparation for visits.
Ensure that barriers to accessing specialty care are identified and addressed including transportation language needs and
insurance issues.
Ensure completion of referral process by obtaining consultant notes and authorizations for additional procedures being
recommended.
Provide education and counseling on the referral program process.
Other Responsibilities
Exercise appropriate judgment and decision-making.
Participate in quality improvement initiatives related to referral management providing feedback and suggestions to streamline
processes and enhance patient care.
Assist with data auditing tasks.
Participate in staff meetings agency meetings planning meetings and other meetings as needed.
Comply with requirements of OSHA HIPAA and other applicable regulations.
Other duties as assigned.
JOB QUALIFICATIONS AND EXPERIENCE:
Medical Assistant certification from approved training program and/or specialty care referral and utilization managed care
experience
Minimum 3 years experience in an outpatient healthcare setting. HIV/AIDS care experience is a plus with basic knowledge of
infectious diseases.
Knowledge of ICD-9/10 codes and CPT coding.
Strong communication organizational and interpersonal skills with a high level of attention to detail.
Proficient in Microsoft Office EHR and other healthcare software.
Self-motivated with the ability to handle multiple tasks and maintain patient confidentiality.
Able to work independently with minimal supervision.
Bilingual (English/Spanish) is a plus.
Experience working with or knowledge of the LGBTQ community and related issues.
