Terros Health is hiring a full-time Family Nurse Practitioner for our Stapley Health Center location in Mesa AZ.

Full-time: 4-10s or 5-8s

4 x 10s (Monday-Thursday): 7:30am-6pm or 8am-6:30pm; or 5 x 8s: 8am-4:30pm or 8am-5pm

(Option for 3-10s 30 hours/week)

AZ FNP License Required

Certification Required

FNP Experience Preferred

NextGen Experience is a PLUS

Salary Range (based on 40 hours/week): MSN: $113K-$118K DOE; DNP: $129K

Additional Language Differential Pay Available: SpanishVietnamese Korean Arabic Farsi Swahili or ASL: $3/hour ($6240 annually)

The Primary Care Providerconducts physicals/check-ups medical evaluations diagnostics treats illnesses orders lab tests provides general and preventative care prescribes and monitors medications coordinates with other providers makes specialty care referrals educates patients about preventative care establishes therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate treatment goals within the context of an integrated setting.

Duties include:

  • Performs independently or as a part of a treatment team.
  • Conducts physicals/check-ups evaluations diagnostics treats illnesses orders lab tests general and preventative care medication prescribing and monitoring and individual treatment and case consultation.
  • Performs the important task of educating patients about preventative care wellness activities and prescribed treatments.
  • Provides leadership and cohesion within the clinic environment and may provide direct supervision to other professional and/or non- professional medical staff and students.
  • Participates in program evaluation procedures and professional record keeping makes referrals appropriately for specialty and behavioral health care.
  • Provides direct patient services not less than 85% of the work time
  • Employs therapeutic interventions appropriate for identified treatment issues
  • Seeksappropriate clinical supervision/support when necessary to ensure the provision of effective treatment
  • Establishes appropriate patient files that conform to agency guidelines best professional practice and State licensing regulations

Qualifications

1. Nurse Practitioner AZ License
2. Masters degree or Doctorate in nursing (Family Nurse Practitioner)
3. Possess a valid BLS certification
4. Possess a valid DEA license and Board Certification (ANCC)
5. Must have valid Arizona drivers license be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Healths driving policy
6. Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
7. Must pass a Drug Screen TB Test and Background Check.
8. Proficient using computer-based systems including electronic health records (NextGen preferred) Microsoft Outlook and other Microsoft Office products.
