Northwestern Counseling & Support Services Inc. is seeking two compassionate organized and collaborative Care Coordinators one to support our Outpatient Team and one in our Access these roles youll be at the heart of helping individuals navigate services build stability and achieve their goals.
As a Care Coordinator youll partner closely with individuals families and interdisciplinary teams to ensure care is thoughtful connected and driven by each persons preferences. This is meaningful relationship-based work for someone who thrives in a dynamic environment and believes in meeting people where they are.
Responsibilities include:
Developing and maintaining individualized care plans in collaboration with clients and treatment teams
Coordinating services across internal programs and community partners
Supporting transitions of care including hospital discharges and follow-up after crisis stabilization
Monitoring progress and adjusting care plans as needs evolve
Collaborating with individuals and their supports to identify preferences and develop crisis plans including education on the National Suicide & Crisis Lifeline and local resources
Assisting with Advance Directives including psychiatric advance directives when appropriate
Proactively following up with missed appointments and addressing barriers to engagement
Tracking utilization data and contributing to quality improvement efforts
