Hospice Registered Nurse Case Manager

Salary: $80-89k/yr BOE

Schedule: M-F 8:00AM-5:00PM (on-call)

Location: Toledo Ohio and surrounding area

Requirements of the RNCM role:

• Licensed Registered Nurse

• BLS Certified

• Comfortable working in both private homes and facility settings.

• Valid Drivers License and Auto insurance Coverage

• Reliable transportation

• A minimum of 1 year of Clinical experience

• Hospice experience preferred

About the RNCM role:

• Conduct comprehensive patient assessments including vital signs and medical history review

• Travel to patients residences to conduct visits

• Develop individualized care plans in collaboration with multidisciplinary teams

• Coordinate hospice care by working with other team members as well as with the patients family members to ensure needs are met.

• Perform evaluations for hospice care along with informational visits as needed.

• Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.

• Educate patients and families on treatment plans medication management and health maintenance strategies

• Submit complete accurate and relevant clinical notes promptly.

• Participate in IDG and case conferences

• Performs on-call duties per policy and as needed

• Conducts an initial and ongoing comprehensive assessment of the patients

• Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.

• Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.

• Provide care plan to Hospice Aide and LPN as part of the IDG team.

• All other duties as assigned
