Position: Behavioral HealthClinical Patient Navigator

Classification: Non-Exempt

Reports to: Director of Behavioral Health

Location: Denver OfficeClinic(occasionaltravel)

Salary Rate:

Level 1 (Entry level up to 2 years of direct experience):$24.04per hour

Level II (3 years of direct experience):$25.19per hour

Bilingual premium for qualifying languages is 3.5% higher than starting salary ifpassbilingual exam on first week of hire.

Position Summary:Under the supervision of theDirector of Behavioral Health and Wellness this position provides an assessment of eligibility insuranceassistanceand health education to clients seekingbehavioral healthservices throughColorado Health Network Behavioral Health Navigation services will support CHN to better serve those disproportionately at risk for or living with HIV/HCV by improving their access tomental and behavioralcare.

Essential Job Functions:

  • Provide screeningsoutreachand initiatives to promoteclinical programing

  • Coordinate andmaintainthe behavioral health waitlist

  • Provideappropriate patientreferrals to both internal and external agencies

  • Determineeligibility of services forBehavioralHealthpatient as they are referred

  • Screen andSchedulingBehavioralHealthoutpatient sessions

  • Provide psychoeducationas needed

  • Assessformental health needsacuity andappropriate placement

  • Liaison between patient internalsystemsand external insurance provider

  • Navigating billing landscape and sharing knowledgewithproviders

  • Performmental healthassessmentscreate plans andassistpatients with access tocaremedicationand adherence to treatment as well as risk reduction plans

  • Responsible for trackingmonitoringand occasional reporting of patient activities including access adherence

  • Maintain accuracy and timeliness of laws/policiesimpactingbehavioral health at CHN

  • Complete necessary paperwork and data entry ina timelyfashion including entering case notes

  • Collect necessary documentationrequiredfor program compliance

  • Assistpatients as they move through the service delivery system

  • Assistwith the elimination of barriers including but not limitedto:insurance financialhousing and transportation

  • Provide emotional support for patients and their families as needed

  • Act as a liaison between patient and all relevant professionals programs and resources

  • Educate patients and others about HIV disease process and prevention

  • Assistwith and coordinate community education and outreach initiatives as requested

  • Maintain and upkeep of electronic health records (EHR)

  • Participate and attend staff departmentteamand community meetings upon request

  • Perform effectively within the organization and interpersonally with co-workers

  • Develop positive working relationships with other social service medical and behavioral health providers

  • Participate in fund-raising and advocacy events

  • Must be available to work evenings and weekends

  • Must have current and valid Colorado drivers license; must be able to pass driving recordcheckper CHN insurance requirements.

  • Must be able to be insured by CHN driving and liability insurance companies.

  • Must have personal insurance onownvehicle if driving on behalf of CHN.

  • Must be able to work in a diverse setting with diverse populations including persons living with HIV the LGBTQ community persons of various ethnic backgrounds disenfranchised communities.

  • Participation in training related to HIV substance use harm reduction trauma informedcareand related issues. Cultural Competency/Sensitivity.

Qualifications

Qualifications:

Comfortability navigating Electronic Health and other experiences may fulfill this perform this job successfully an individual must be able to perform each essential duty satisfactorily. Education:

Bachelors orMastersDegreeinsocial work sociology psychology counselingpublic health or nursing; And/or equivalent combination of education and experiencerequired.

Software Utilized:

Proficiencywith Microsoft Office applications such as Word Excel Outlook and Teams isrequired. Candidates should be comfortable working with client database systems and learning new software tools as needed.

KnowledgeSkilland Ability:

Mental health and harm-reduction knowledge of HIV STIsPrEPand other prevention methods. Proactive problem solver who works well in multi-disciplinarysettings. Ability to communicate well with medical providers and support staff. Strong verbal and written communication socio-cultural identification or work experience with MSM and LGBTQ preferred. Comfort discussing sexual health and risk reduction strategies. Proven organizational skills with high attention to detail. Ability to work well with diverse populations. Ability to work well within the organizational in the delivery of evidenced-based programs is highly preferred as well as excellent communication client services and data collection personwill be highly organized position requires personal and professional accountability; self-management and initiative; dedication to quality improvement; professional integrity; research ethics and cultural superior organizational in the following areas: task and time management; teamwork; and communication with teams managers clients and participants and company consultants.

Ability to speak Spanish desired.

Material and Equipment Directly Used:

Works with standard office equipment including computerwork station printers copiersscannersand faxmachine. Works with word processing software spreadsheet applications and presentation software. Communicates with clients and vendors using the telephone e-mail the Internet and fax.

Physical Demands and Work Conditions:

Works in an office environment. May travel via automobile or airplane to offsite locations or for CHN business. Must have andmaintaina valid Colorado drivers license and auto insurance on personalvehicle. Works with standard office equipment including computerwork station printers copiersscannersand faxmachine. Communicates with clients and vendors using the telephone e-mail the Internet and clear speaking ability and the ability to use a telephone for extended periods. Requiresabilityto view a computer screen (near acuity). Requires fingering agility for ability to sit stand and engage in extended periods of screen time or communication by phone is necessary to perform the essential duties of the role.
