We are looking for a skilled Social Worker to join the Adult Mental Health North Team. Mental Health Social Care Teams are specialist teams that commission and review a wide variety of packages of care for citizens who are in contact with secondary mental health services with the aim of promoting independence and wellbeing.

What Youll Be Doing

  • Carrying out strengthsbased assessments reviews and care planning under the Care Act 2014.
  • Leading and contributing to Section 42 safeguarding enquiries.
  • Mental Health Tribunal Hearings and writing Social Circumstance Reports
  • Working closely with AMHPs health partners housing and community providers.
  • Supporting individuals with complex presentations including psychosis personality disorder trauma and substance misuse
  • Managing a varied caseload including hospital discharges residential placements community support packages and crisisdriven work.
  • Attending multiagency meetings such as MAPPA MDTs and risk panels.

About You

  • Qualified Social Worker (SWE registered).
  • Previous mental health experience experience of working with citizens who are open to secondary mental health services is desirable.
  • Strong understanding of the Care Act Mental Capacity Act and safeguarding frameworks. Understanding of the Mental health Act is desirable.
  • Confidence in managing risk and working autonomously
  • Excellent communication skills and the ability to work collaboratively with a wide range of professionals.
  • A reflective practitioner who values supervision learning and continuous development.

A DBS enhanced check for a regulated activity is required for this post.
