Community Intervention Team (CIT)
The Community Intervention Team (CIT) is a community based acute nursing service providing fast tracked acute nursing support for a finite period to adult patients in Kildare West Wicklow both in their homes and the CIT Clinics which is based in Nass- St Marys
The service accepts referrals from acute hospitals GPs Public Health Nursing services and Primary Care based health professionals.
The aim of the service is to facilitate early discharge of suitable patients from hospitals work collaboratively with community based Integrated care teams and GPs to support avoidable hospital admissions where possible and to maximize clients living with chronic disease to be cared for as much as possible in the community setting.
The service operates between 8am to 8pm seven days per week. Nurses are rostered to work over a 7-day week with an option for nurses to work long or short days.
Home services CIT deliver include administration of I.V antibiotics as part of the national OPAT programme. Subcutaneous fluids home phlebotomy Oncology care Renal assessment care INR monitoring medication compliance. Health education and support to service users with chronic illnesses. Nurses are required to use their own cars for home visits and mileage allowance is payable at HSE travel allowance rates.
