Registered Nurse RN Home Health Case Manager

What You Will Do

Manage a consistent caseload from admission through discharge
Complete Start of Care OASIS recertifications and routine visits
Develop implement and adjust patient-centered care plans
Identify clinical changes early and intervene appropriately
Educate patients and caregivers to improve independence
Collaborate with physicians and interdisciplinary teams
Document efficiently using HomeCare HomeBase

What You Bring

Active RN license
1 year clinical nursing experience
Home Health or Hospice experience preferred
Strong assessment organization and time management skills
Ability to work independently and make clinical decisions
Reliable transportation and willingness to travel within territory

Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
HomeCare HomeBase experience is a plus
