• Day Shift: 6:00 AM 6:30 PM
• Night Shift: 6:30 PM 6:00 AM

Position Overview

Navitas Healthcare LLC is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in a structured healthcare setting. The LPN will support residents through medication administration treatments and coordination with the healthcare team while ensuring compliance with state and facility standards.

Key Responsibilities

• Administer medications (oral enteral parenteral etc.)
• Perform treatments (wound care trach care catheter care dressing changes)
• Monitor patient response and report changes in condition
• Maintain medication and supply availability
• Participate in shift handoffs and patient reporting
• Communicate with residents families and care teams
• Conduct initial assessments for new admissions
• Assist with MDS care planning and reviews
• Maintain accurate documentation of patient care
• Participate in QAPI and quality improvement initiatives

Required Qualifications

• Active Arizona LPN License
• Strong clinical judgment and emergency response ability
• Ability to assess document and report patient conditions
• Strong communication and teamwork skills

Preferred Qualifications

• Experience in geriatric or long-term care settings
• Familiarity with care planning and MDS processes
