Job Summary:
We are seeking a dedicated Licensed Practical Nurse to provide high-quality care in a long-term care and rehabilitation setting. The role involves medication administration patient monitoring and supporting residents with chronic and cognitive conditions in a structured care environment.
Key Responsibilities:
- Administer medications and perform medication pass (Med Pass) duties
- Provide direct patient care in a long-term care setting
- Monitor and support residents with Alzheimers and dementia
- Assist with daily living activities and patient safety
- Document care accurately using facility systems
- Collaborate with nursing staff to ensure quality resident care
Facility Overview:
- Approximately 300-bed long-term care and rehabilitation center
- Patient population includes Alzheimers and dementia care residents
Requirements:
- Active New York State LPN/LVN license required (must be in hand at submission)
- Minimum 1 year of nursing experience required
- Long-term care experience preferred (rehab med-surg or hospital experience also accepted)
- BLS certification required
- COVID-19 vaccination required
- First-time travelers accepted
- Must be willing to work every other weekend
