Job Summary
Job Title: Registered Nurse Case Manager
Location: Anchorage AK
Type: Contract
Shift: Day Shift (7:00 AM 4:00 PM or 8:00 AM 4:00 PM)
Schedule: 5x8 Hours (Every Other Weekend Required)
Weekly Hours: 72 hours per 2 weeks
Position Overview:
Navitas Healthcare LLC is seeking Registered Nurse (RN) Case Manager for an exciting Travel or Local job in Anchorage AK.
Responsibilities:
- Coordinate and manage patient care plans across continuum of care
- Perform patient assessments for care level and discharge planning needs
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Facilitate discharge planning and post-acute care coordination
- Monitor patient progress and update care plans as needed
- Ensure compliance with hospital and regulatory guidelines
- Document and track care using Epic EMR
- Advocate for appropriate utilization of healthcare services
- Communicate with patients families and care teams
- Support quality improvement and care management initiatives
Requirements:
- Active Alaska RN license (required)
- Minimum 1 year acute care RN experience
- Epic EMR experience required
- BLS certification required
- Strong clinical assessment and care coordination skills
- Ability to work in fast-paced hospital environment
