Job Purpose:
The role of the RespRedirect Clinical Nurse Specialist in Respiratory Medicine is to provide efficient and effective care for all patients and their families. S/he will be a key member of the Multi-Disciplinary Team involved in the assessment and management of their care. The RespRedirect CNS aim is to support patients from the Emergency Department setting to avoid hospital admission where possible. The post holder will engage in timely patient reviews and act as a role model demonstrating clinical expertise relating to respiratory disease care. S/he will actively promote and develop holistic care plans for each patient who attends the service. The post holder will be responsible for coordinating the care of Respiratory patients referred to MMUH from external partner sites.
The Clinical Nurse Specialist (CNS) for the Respiratory Redirection Service will provide strategic and clinical leadership in establishing developing and sustaining an innovative service that optimizes respiratory care pathways and reduces avoidable hospital admissions. This role represents an exciting opportunity for a dynamic nurse leader to shape and embed a model of care that enhances patient outcomes promotes integrated multidisciplinary collaboration and supports long-term service sustainability across the Mater Misericordiae University Hospital (MMUH) and its partner sites.
The post holder will lead the coordination and delivery of timely evidence-based assessment intervention and redirection for patients presenting with respiratory conditions. Acting as a clinical expert and change agent the CNS will work collaboratively with multidisciplinary teams (MDTs) community partners and external stakeholders to ensure seamless patient-centered care across hospital and community settings.
Through innovation education and continuous quality improvement the CNS will drive the development of holistic evidence-informed care plans and contribute to the evolution of sustainable respiratory services that optimize patient outcomes and resource utilization
