Job Summary
Job Title: Travel Social Worker
Location: Cumming Georgia
Duration: 13 Weeks (Possible Extension)
Shift: Monday Friday 12:00 PM 8:30 PM
Pay Rate: $1900 $2200/week (Based on Experience)
Position Overview
We are seeking an experienced Travel Social Worker to provide comprehensive support to patients and families in a hospital-based setting. This role focuses on care planning emotional support crisis intervention and connecting individuals with appropriate community resources.
Key Responsibilities
Assess clients needs situations and challenges
Develop and implement individualized care and treatment plans
Provide counseling and emotional support to patients and families
Assist clients in coping with stress trauma and life transitions
Connect individuals with community resources and support services
Advocate for clients rights safety and overall well-being
Monitor and evaluate client progress and adjust care plans as needed
Maintain accurate and timely documentation of case records
Collaborate with healthcare providers and interdisciplinary teams
Provide crisis intervention and support during emergency situations
Required Qualifications
Minimum 2 years of hospital-based social work experience
Active LCSW license in the state of Georgia
BLS (Basic Life Support) certification
Strong interpersonal communication and counseling skills
Ability to manage multiple cases in a fast-paced environment
