Patient Care Coordinator / Patient Support Associate (Order Management & Clinical Documentation)

Location: Remote (Philippines)
Schedule: US Hours (Full-Time) 40hrs/week Mo-Fr
Compensation: $1800$2000/month performance bonus/commission

About the Role

We are seeking a detail-oriented Patient Care Coordinator / Patient Support Associate to support a US-based home health and hospice platform. This role is heavily focused on physician order management clinical documentation accuracy and ensuring billing readiness through complete and audit-ready records.

This is a high-accountability role requiring strong English communication precision and the ability to work in structured healthcare systems.

Key Responsibilities

Physician Order Management (Core Focus)

• Receive process track and reconcile physician orders
• Follow up on unsigned missing or incomplete orders
• Ensure documentation is complete and ready for billing submission
• Maintain audit-ready medical records at all times

Clinical Documentation Support

• Review medical documentation for accuracy and completeness
• Communicate with clinicians regarding corrections or missing information
• Upload index and manage EMR documentation

Remote Patient Monitoring (RPM) Support

• Assist with patient onboarding into RPM programs
• Route alerts and updates to appropriate clinical teams
• Maintain accurate RPM documentation records

Required Skills

• Strong English communication (written and verbal)
• High attention to detail and accuracy in documentation
• Experience with EMR systems or healthcare platforms preferred
• Comfortable working in structured compliance-heavy environments
• Strong follow-through and ownership mindset
