What Youll Do

• Manage a consistent caseload of patients in their homes

• Perform comprehensive assessments and develop individualized care plans

• Provide hands-on skilled nursing care and patient education

• Monitor progress and adjust care plans to ensure the best outcomes

• Collaborate with physicians therapists and the broader care team

• Build meaningful relationships with patients and families during critical moments in their healthcare journey

What Youll Need

• Active unrestricted RN license

• At least 1 year of nursing experience (home health experience is a plus not a must)

• Strong clinical assessment and critical thinking skills

• A valid drivers license reliable transportation and auto insurance

• Willingness to travel within your assigned territory

Physical Requirements

Sit stand bend lift and move intermittently and be able to lift 50100 lbs.
