Family Nurse Practitioner AVALA Care Full-Time RO49

Summary

The Family Nurse Practitioner is responsible for caring for patients maintaining accurate and current patient records and scheduling as well as administering follow-up appointments to patients as required. They work as a team with our nurses/medical assistants and physicians in delivering premium care to patients.

Essential Duties and Responsibilities

• Conduct comprehensive patient assessments including medical histories and physical examinations
• Diagnoses and manage acute and chronic health conditions
• Develop and implement individualized treatment plans in collaboration with supervising physicians
• Order perform and interpret diagnostic tests and laboratory results
• Prescribe medications and therapies in accordance with state regulations
• Educate patients on disease prevention treatment plans and medication management
• Document all patient encounters accurately in the electronic health record (EHR)
• Coordinate care with physicians specialists and other healthcare professionals
• Monitor patient progress and modify treatment plans as necessary
• Ensure compliance with HIPAA infection control and organizational policies
• Participate in quality improvement initiatives and continuing education
• Perform other duties as assigned by leadership

Professional/Regulatory Requirements

• 3 years of Family Nurse Practitioner experience preferred
• Masters or Doctoral degree in Nursing from an accredited program
• Current certification as a Family Nurse Practitioner (FNP-C or FNP-BC)
• Active and unrestricted Louisiana APRN license
• Prescriptive authority and DEA registration
• Current BLS certification (ACLS preferred)

Skills

• Ability to communicate effectively in English both verbally and in writing.
• Basic computer knowledge.
