Job Summary:
We are seeking an experienced RN Case Manager to support discharge planning and care coordination in an acute care setting. The ideal candidate will have a strong background in case management and the ability to manage a high patient caseload efficiently.
Key Responsibilities:
- Coordinate patient care and discharge planning
- Collaborate with interdisciplinary teams to ensure appropriate care transitions
- Assess patient needs and develop individualized care plans
- Monitor patient progress and adjust plans as needed
- Communicate with patients families and healthcare providers
- Maintain accurate documentation in EMR systems
Patient Load:
- Approximate ratio: 1:20
Requirements:
- Minimum 2 years of acute care case management or discharge planning experience
- Active California RN license required (pending license accepted but must be active by start)
- First-time travelers accepted (subject to screening)
- Strong communication and organizational skills
