Job Summary: We are seeking an experienced RN Case Manager to support discharge planning and care coordination in an acute care setting. The ideal candidate will have a strong background in case management and the ability to manage a high patient caseload efficiently.
Key Responsibilities:
Coordinate patient care and discharge planning
Collaborate with interdisciplinary teams to ensure appropriate care transitions
Assess patient needs and develop individualized care plans
Monitor patient progress and adjust plans as needed
Communicate with patients families and healthcare providers
Maintain accurate documentation in EMR systems
Patient Load:
Approximate ratio: 1:20
Requirements:
Minimum 2 years of acute care case management or discharge planning experience
Active California RN license required (pending license accepted but must be active by start)
First-time travelers accepted (subject to screening)
Strong communication and organizational skills
