Job Summary

Minimum Qualifications

  1. Education - masters degree in social work required
  2. Experience - Experience working with the population served by the clinic/program strongly preferred

Certifications / Licenses

  • Licensed by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
  • Current valid NYS registration certificate
  • Must be eligible for full and unconditional participation in Medicaid and Medicare programs
  • May be required to possess a valid NYS drivers license and clear driving record (depending on assignment)

Must be authorized to work in the U.S.

About the Role

Social Workers establish and maintain a therapeutic relationship with individuals and their families by effectively utilizing professional social work skills. They may review psychiatric medical and other case records; complete admission screenings; conduct psychosocial assessments; make treatment recommendations; participate in treatment planning; provide social casework services; deliver individual group and family therapy; assess availability of services; serve as consumer advocates; develop and implement discharge plans; and coordinate services with community providers. Social Workers advocate to ensure individuals receive appropriate treatment and services that their legal and human rights are protected and that services and resources are utilized consistently and effectively.

Primary Responsibilities:
Assessment and Treatment Planning
  • Interview individuals following review of records to complete psychosocial assessments
  • Obtain psychosocial histories by interviewing individuals families significant others and former service providers
  • Assess and prioritize treatment needs and determine appropriate levels of care when applicable
  • Participate in the development review and updating of individualized treatment plans
  • Ensure referrals for ancillary support and consultative services are identified and made
  • Ensure continuation of care is arranged and documented prior to discharge

Clinical and Therapeutic Services

  • Provide individual group and family therapy
  • Provide supportive and supplemental counseling particularly during crisis situations
  • Conduct group lifeâskills training treatment groups and counseling sessions
  • Prepare outlines objectives and facilitate sessions according to treatment schedules
  • Evaluate effectiveness of group sessions and modify as needed

Discharge Planning and Care Coordination

  • Assess readiness for discharge and identify needed resources
  • Coordinate services with community providers and conduct followâup as needed
  • Educate individuals and families on illness treatment and available services
  • Identify and assist with planning for financial and social support needs when appropriate

Documentation and Team Collaboration

  • Prepare progress notes treatment recommendations reports and discharge plans within required timeframes
  • Ensure treatment team members are informed of individuals clinical and psychosocial needs through documentation
  • Participate as an integral member of the interdisciplinary treatment team

Family Services

  • Provide treatment to family members and significant others or advocate for such services
  • Promote and increase family involvement in treatment programs
  • Facilitate family education sessions in alignment with treatment plans

Supervision (When Assigned)

  • May supervise social work students
  • Meet with staff regarding caseloads and performance issues
  • Complete performance programs evaluations scheduling and attendance oversight
  • Maintain liaison with educational institutions to ensure highâquality student clinical experiences

Essential Knowledge Skills and Abilities

  • Ability to work with individuals families and staff in a respectful and culturally responsive manner
  • Ability to develop implement evaluate and modify treatment plans while meeting deadlines and productivity standards
  • Strong verbal and written communication skills
  • Ability to prepare accurate and timely documentation using assigned computer systems and software
  • Knowledge of and adherence to internal and external laws rules and policies
  • Ability to work independently and collaboratively while following safety and accountability measures
