Must have knowledge of general cardiovascular critical care particularly peds exp managing ECMO/CRRT/Bags. No exceptions can be made if the provider does not have a master s degree. Nurse Practitioner: NP board certifications are outlined in the job description. No exceptions can be made if the provider does not have the board cert listed. DEA : CA DEA with Schedule 2/2n required. Certifications : BLS required for all positions additional certifications will be listed in the job description. ALL certifications must be AHA. Malpractice and disciplinary background issues: If your provider has more than 500K in malpractice or more than 2 cases please include first and third party documentation on the cases. Credentialing/Privileging: Privileging generally takes 60-90 days. It s imperative that the provider is available to send their DOB SSN License contact information and a signed form sent during the offer process within 72 hours of signing. This allows for the facility to kick off privileging and any delays will impact start time. No emergency privileges. Parking: The facility does not offer parking passes. Parking costs will need to be covered by the provider. There is public transportation available through the BART with shuttles going to each campus on a regular basis.
