Job Summary
Prevention Intensive Case Manager
As a Prevention Intensive Case Manager you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for children with multiple stressors provide a safe environment to return children already in placement and prevent future out-of-home placement. You will report to the Program Coordinator and work within a treatment team including a Clinician and/or a Community Support Worker depending upon the needs of each family. You will work a combination of on-site and remote with travel to clients homes required.
Responsibilities:
Provide home and community-based intensive wrap- around case management services (the level of intensity / number of direct services hours per family is dependent upon the program/ services in which one is employed.
Services to be provided in the home and community include the following and are dependent upon the needs of each family: parenting skills training; household management; emotional regulation/stress management; problem- solving training; development of behavior management plans for home and school; supervised visitation; and therapeutic recreation.
Work in partnership with a Clinician and/or Community Support Worker to provide services and support to each client family following the goals of their individual treatment plan.
Requirements and Education:
Bachelors degree in Social Work Sociology Criminal Justice Psychology Human Services or a related field required.
2 or more years of post-degree experience in Human Services
Home and community-based experience is a plus.
A valid drivers license is required.
