Position Overview
Navitas Healthcare LLC is seeking a Licensed Master Social Worker (LMSW) to support case management services in a Level II Trauma Center. The role focuses on discharge planning care coordination and providing psychosocial support in a fast-paced acute care environment.
Key Responsibilities
- Coordinate discharge planning and care transitions
- Conduct psychosocial assessments and provide support
- Manage caseloads across hospital units
- Collaborate with interdisciplinary teams
- Maintain accurate documentation in Epic EMR
Required Qualifications
- Active Washington State LMSW License
- Minimum 1 year of acute care experience
- Experience in discharge planning and care coordination
- BLS Certification required
Preferred Qualifications
- Experience in Med-Surg ED and LLOS cases
- Strong communication and organizational skills
- First-time travelers accepted
