PathPoint is seeking a full-time Medical Facilitator in Santa Barbara CA 93101. The Medical Facilitator oversees client medications provides rehabilitation services and individualized support for adults with behavioral health disorders. Additionally they serve as the primary counselor or co-counselor for assigned clients.
This is a full-time position for 40 hours per week Monday-Friday between 7:45am and 5:30pm.

What Youll Do:
- Orders clients medications Requests PRN authorizations and communicates with psychiatrist or primary care doctor for clients to self-determine their use of as-needed medications. Observes and facilitates clients taking medications as prescribed. Documents medication monitoring.
- In coordination with clients develops implements and updates individual Action Plans.
- Provides ongoing assessment of clients mental illness symptoms and responses to treatment. Recognizes and is available to clients in psychiatric crisis or emotional distress.
- Assists clients in developing appropriate coping conflict resolution and problem-solving skills. Provides counseling emotional support and symptom recognition and management.
- Assists clients with healthcare benefits scheduling medical appointments maintaining medical records learning household maintenance personal hygiene shopping and cooking skills money management skills and accessing reliable transportation.
- Functions as a positive role model and promotes cooperation and harmony in communal living.
Who Were Looking For:
- Bachelors degree in a Psychology or a related field and one year of experience in the mental health field; OR high school diploma/GED and at least 5 years of relevant experience
- Must be legally able to bill Medi-Cal
- Knowledge of CCL regulations preferred
- Teamwork - able to balance team and individual responsibilities
- Client-centered focus - Assesses program services to assure that client needs and goals are reflected in the activities
- Professionalism - Treats all with respect and works well under pressure
- Judgement - able to make decisions and demonstrate sound and accurate judgements in a timely manner
- Communication - excellent written and oral communication
- Planning & Organization - excellent time management skills
- Must have a valid California drivers license with a satisfactory driving record 5 years of driving experience and be eligible for coverage on PathPoints auto insurance policy.

Compensation: This is an hourly non-exempt position with a starting wage of $31 per hour. The pay range for this position is $31-$34.50** an hour.
**Wages subject to change based on a variety of factors and the maximum pay represents the wage potential based on attaining the highest level of position experience talent and responsibilities
